Eligibility Verification

OUR CUSTOMER SERVICE REPRESENTATIVES ARE EAGER TO PROVIDE ASSISTANCE.

To obtain approval for supplies covered by Oklahoma Medicaid, certain criteria must be met. Firstly, a Face to Face consultation or a Tele Health appointment with your contracted Medicaid Provider is necessary. Your Medicaid Provider will be requested to complete a set of forms that we will promptly dispatch prior to your scheduled consultation.

Documentation Needed:

1. Evidence, within the past six months, substantiating the medical necessity of the required items as determined by your Medicaid Provider.
2. The specific medical condition warranting the need for these supplies.
3. An estimation of the daily quantity of supplies required.

Once we have gathered your documentation, we will proceed with the submission of your application for Prior Authorization to Oklahoma Medicaid. Upon successful approval of your Prior Authorization, we will ship the supplies directly to you.
